FULEIHAN Septorhinoplasty Set

Rhinoplasty in general and endonasal rhinoplasty in particular require a good exposure of the elements that require reduction, rearrangement or augmentation. Special instruments are needed to achieve exposure of different areas under correction. Instrument design plays a key role in undermining, exposure and tissue handling. The dynamics of retracting tissues, seeing the lower lateral cartilage and working on the dorsum with a better view of the bony cartilaginous framework affect the level of precision on this surgery.

Nevertheless undermining mucoperichondrium and mucoperiosteum is the main step in septal surgery. Avoiding multiple changes in instruments saves time and improves concentration on the surgery.

By the same token having instruments that can handle cartilage and avoid damaging and permitting its reshaping is essential during rhinoplasty.

The FULEIHAN Double-Retractor is specifically designed for retraction of the alar skin in order to expose the alar cartilage. It is dynamically designed to permit digital manipulation of the cartilage to see its endonasal surface as well as the rim without the need of frequent repositioning facilitating its exposure and surgical manipulation.

Characteristics and functions of this instrument:

  • Light weight.
  • Wide opening of the retractor site where digital pressure is applied permitting better exposure of the alar cartilage and manipulating it.
  • The hooks on both sides are semi-blunt permitting good grasp of the sensitive alar rim without traumatizing it.
  • One side has a two hooks directed diagonally.
  • The other side has three hooks permitting homogeneous alar rim retraction.
  • The middle portion contains different openings that can assist in shaping shield cartilage grafts for tip definition. Two size shield graft templates are present.
  • The edges of the instruments are marked with measurements scales.

The FULEIHAN Nasal Retractor is designed to give a better exposure of the bony cartilaginous vault. The depth and width permit easier reduction, cartilage positioning and placement of sutures.

Characteristics of this instruments:

  • Wide and deep flange that can give a better view of the dorsum.
  • The retractor has an elliptical hole that permits placement of sutures from outside and inside for fixation of dorsal and lateral grafts. It also allows the insertion of sutures for suspension of the upper lateral cartilage
  • The hole in the retractor permits to localize the deformity and irregularity with better precision. When the deformity is mapped on the skin, it is translated through digital pressure endonasally by seeing the site through the hole and the exact location of the bony cartilaginous deformity is localized
  • The other side has a retractor with a equal to the base of the tip for retraction the tip skin in an open Rhinoplasty

Handling cartilage without traumatizing it and having a good purchase of the cartilage so that it does not slip is essential. The FULEIHAN Forceps is designed in order to have a better grasp of the septal cartilage and be able to graft insert fixation sutures while the graft is held in position.

Characteristics of this instrument:

  • The teeth of the forceps are widely spaced in order to decrease trauma to the cartilage and at the same time avoid slipping
  • The forceps has a wide central hole that permits the insertion of fixation sutures in the cartilage while held in position
  • The hole is essential to pass the needle between the two edges of the forceps or within the body of the graft
  • This instrument can also be used to fix the septum, mucoperichondrium and vestibular skin to insert the transfixion suture or to fix a septal splint

The FULEIHAN Elevator is designed in order to facilitate elevation of the mucoperichodrium and mucoperiosteum without the need to change instruments, thereby improving the efficiency of the surgery.

Characteristics of this instrument:

  • One end is designed to elevate mucoperichondrium during a septoplasty. It has a round thin head that permits easy elevation of the mucoperichondrium
  • The other end is thicker with a sharp edge permitting easier elevation of the mucoperiosteum of the maxillary crest and vomer region.
  • This is a combination of a Cottle and Joseph elevators in one Instrument
